Note: This form should only be completed by authorized brokers who have provided the training to a prospective small business.

Brokers who have met with a prospective small business should use this form to confirm that they have discussed coverage options through MHC or MHC for Small Business with the employer. Completing this form is the final step for the employer to receive a seal demonstrating their commitment to providing coverage for their employees.

Please note that false attestations will result in the seal not being issued to the employer, and brokers may face disciplinary action for participating in this program.
