The information on my 1095-A doesn’t look correct. How can I get it fixed?

If you think there is a mistake on your Form 1095-A, you should call Maryland Health Connection at 855-642-8572 (TTY: 855-642-8573).

Please be prepared to explain which sections of the form look wrong and why. If you have other documents, such as your bill from your insurance company, please have them available for reference. You should note that the premium total listed on your 1095-A form may not match exactly with your billing records, if your plan covered non-essential benefits such as adult dental or vision.

Note: The research process cannot be completed while you are on the phone and may take several business days. Once we complete researching your case, we will notify you or send you a corrected Form 1095-A.
