If you think there is a mistake on your Form 1095-A, you should call Maryland Health Connection at 855-642-8572 (Deaf and hard of hearing use Relay service).
Please be prepared to explain which sections of the form look wrong and why. If you have other documents, such as your bill from your insurance company, please have them available for reference. You should note that the premium total listed on your 1095-A form may not match exactly with your billing records, if your plan covered non-essential benefits such as adult dental or vision.
If you had a plan through us last year and did NOT receive a tax credit, you can still use your 1095-A to file taxes, even if the form has an incorrect premium amount. The form just serves as your proof of health coverage for the year.
