Maryland Health Connection will mail Form 1095-A by Jan. 31 to the mailing address on file of any individual or family who enrolled in health insurance through Maryland Health Connection for any period of time in the last tax year.
Your Form 1095-A is also available electronically in your online account at MarylandHealthConnection.gov. Click on your account's "My Inbox," go to Message Center — View More, and scroll to the bottom.
If you need a replacement copy of your 1095-A, did not receive it, or need a copy sent to a new address, please call 855-642-8572 (Deaf and hard of hearing use Relay service).
Please note, only the primary tax filer(s) you listed on your the last year's application with Maryland Health Connection will be sent a 1095-A. If you did not list yourself as a primary tax filer when you applied to Maryland Health Connection, please ask the primary tax filer in your household for a copy of the form.
