Why did I receive more than one Form 1095-A?

If you or a member of your tax household enrolled in more than one health plan, you will receive a Form 1095-A for each plan. You may also receive more than one form if you reported a life event or if you had a gap or lapse in your coverage in the last year. If there are more than 5 individuals covered by a policy, you will also receive one or more additional Forms 1095-A that continue Part II. You should use all of the 1095-A forms you receive where you or a household member are listed as a “Covered Individual” (in Part II) to fill out Form 8962.
