Your Taxes: Next Steps for Forms 1095-A and 1095-B

IRS Form 1095-A

If you were enrolled in a qualified health plan through Maryland Health Connection at any time this year, there are important steps you need to take when you file your federal taxes.

Save Form 1095-A (Health Insurance Marketplace Statement).

Keep this form with your important tax information, like your W-2 forms and tax records.

  • Form 1095-A is an IRS tax form that serves as proof that you and members of your family met the requirement under the Affordable Care Act to have health insurance coverage. It will be mailed to you by the end of January and is available in your online account.

Check to make sure the information on the form is correct.

Verify the names of household members enrolled, their information and dates they had health coverage.

  • If you think the information shown on your Form 1095-A is wrong (or if you need another copy of the form), please call Maryland Health Connection at 1-855-642-8572 (TTY: 1-855-642-8573).

Use this form to complete IRS Form 8962 (Premium Tax Credit) when you file your taxes.

You should wait until you receive Form 1095-A to file your federal income tax return.

  • If you received tax credits in the previous year through Maryland Health Connection, you must file a federal income tax return, even if you’re not otherwise required to file one.
  • The information from these forms helps make sure you received the right amount of tax credit for your health insurance plan.
  • If you need help determining the Monthly Premium Amount of Second Lowest Cost Silver Plan (Part III, Column B, on Form 1095-A), there is a worksheet available at MarylandHealthConnection.gov/taxes.

You or your household also may receive an IRS Form 1095-B or IRS Form 1095-C.

  • If you or members of your household had coverage through other programs or plans, such as through Medicaid or an employer, you also may be getting a Form 1095-B or Form 1095-C.
  • It’s important to follow the instructions on these forms so you fill out your federal income tax return correctly.

IRS Form 1095-B

If you or a member of your household received Medicaid or Maryland Children’s Health Program (MCHP) health coverage during the previous year, you will receive a new tax form, IRS Form 1095-B. Here are your important next steps:

Save Form 1095-B.

  • Form 1095-B is an IRS tax form that serves as proof that you and members of your family met the requirement under the Affordable Care Act to have health insurance coverage. It will be mailed to you in February.

Check to make sure the information on the form is correct.

Verify the names of household members enrolled, their information and dates they had health coverage.

  • If you think the information shown on your Form 1095-B is wrong (or if you need another copy of the form), please call Maryland Health Connection at 1-855-642-8572 (TTY: 1-855-642-8573).

You or your household may also receive an IRS Form 1095-A or IRS Form 1095-C.

  • If anyone in your household was enrolled in a qualified health plan through Maryland Health Connection, you also will receive a separate Form 1095-A with instructions. You should wait until you receive Form 1095-A to file this year’s federal income tax return.
  • If anyone in your household was enrolled in health coverage through their employer, you will receive a separate Form 1095-C with instructions.
